How the process works:

  1. Fill in your company information below, including the number of people you insure.

  2. We will review your submission and contact you.

  3. After speaking with us, we will send you an invoice, which you can choose to pay by either credit card (fees apply) or ACH via a payment portal.

  4. After paying, you will then receive an email to set up the program for your company by filling out a registration form.

  5. After that, the program will be sent to the person you designate on the registration form and you can start the program!

  6. Check with your insurance account manager to see how to get your payment reimbursed.
